Job Description: This is an entry level, trainee care manager position. Upon successful completion of a 6-month probationary period, employee will be promoted to an Aging Care Manager 2 with salary increase. Aging Care Managers are trained to assess the needs of older adults; develop and implement care plans; provide advocacy and ongoing care management; and protect consumers from abuse, neglect, exploitation and abandonment. Our goal is to improve the quality of life for older adults by providing them with access to a range of community and home-based services.
